Services and Rates

Initial Visit:
Please allow 1.5 hours for initial visit to fill out paperwork, initial interview and treatment. The intake and interview will assist us in identifying your present concerns and discuss your health history. I will tailor a treatment plan specifically targeting your health concerns and goals.
For return office visits please allow an hour to discuss changes in your health and allow for treatment time.

Treatment Plans:
Once we have established your treatment plan, most patients come in weekly or biweekly. Most conditions average 6-12 treatments. Treatment and frequency depend on the severity and nature of the chief complaint.

*For cash only patients, to receive a 10% discount you must pay by cash or check at the time of service otherwise you will be billed for the full treatment price.

Insurance:
I will be pleased to bill your insurance company on your behalf. I am a contract provider for massage and acupuncture with:

I am in the process of becoming an in-network provider with other insurance companies as well. Please call to see if I can submit claims to your insurance. If your insurance covers out of network services, they may cover my services. Contact your insurance company for more information. I will be pleased to bill your insurance company on your behalf.

Cancellations:
Your appointment time is reserved specifically for you. In the event of a missed appointment of cancellation within 24 hours, you will be charged a fee of $20.
